Call for projects - Loto-Voyage

The Cité de la Santé Foundation has been organizing a Loto-Voyage for several years. Each year, the profits from this fundraising activity are devoted to projects submitted by employees of the various departments of the CISSS de Laval. This year, we are granting $100,000 in projects, taxes included. Project applications are made online. Click here for the form.

We invite you to highlight, in the “Description of the request” section, the impact that your project will have on the clientele and/or on your daily work with them. This argument will have an influence on the acceptance of the project. Please note that the “Description of the request” section to be completed is limited to two text boxes of 1000 characters each, for a maximum of 2000 characters. It is important that you know that the projects will be studied by a committee made up of representatives of the CISSS de Laval and the Foundation.

Also, it is essential to discuss your project with your superior. He will be able to find out if a device that you are proposing to purchase can be acquired using the CISSS budget provided for the replacement of an asset. He will also be able to see if a training you offer can be paid for by the CISSS training budget. If necessary, he will check whether installation and maintenance costs apply.

Restrictions

It is important to clearly understand that the role of the Foundation is to improve the care and services offered at the CISSS de Laval. Take note of the following restrictions before completing your application:

Any employee working in one of the CISSS de Laval facilities can apply for Loto-Voyage funding (including the Jewish Rehabilitation Hospital, the CHSLDs, the Louise-Vachon Rehabilitation Center, the Youth Center, etc.)
Proposers must demonstrate the impact of the project on the beneficiaries (and if possible, document it: statistics, needs, etc.).
The projects submitted can be: medical equipment, educational or intervention material or concrete projects/workshops/etc allowing the improvement of the quality of life of the beneficiaries.
No project totaling more than $3,000 taxes included will be considered by the evaluation committee.
All requests for the replacement of furniture, for example chairs in a waiting room, are not eligible.
No project related to training activities will be accepted, since the Foundation grants training scholarships to members of the nursing staff and the Multidisciplinary Council. Contact us for more details.

Procedure

Clicking on the submit a project today link will open a window to the online form. This type of form is connected with our database – THIS IS NOT A DONATION FORM; it will allow us to compile your requests in minutes.

We suggest that you prepare your request on a WORD document and then copy it into the appropriate fields, in particular for the two boxes to be filled in for the description of the request, which are 1000 characters each.

After submitting your online request, step 3 (“Payment method”) you will receive a confirmation email at the email address you provided.

It is very important to remember that your request must total a maximum of $3,000 taxes included. Also note that the deadline to submit an application is Tuesday, May 23, at 4 p.m. You will receive news from the project selection committee before the end of June.

IMPORTANT: Projects must be completed before June 2024.
