The Cité de la Santé Foundation is proud to organize, for several years now, a Loto-Voyage. Each year, the funds raised by this activity are dedicated to projects submitted by employees from various departments of the CISSS de Laval.
This year, we are awarding $104,000 in projects, taxes included. The program is aimed at projects that will promote the autonomy, rehabilitation, integration or social participation of users. Project applications can be submitted online. Click here for the form (available in French only).
We invite you to read the project submission procedure (available in French only) carefully, and to explain in the “Description of request” section the impact your project will have on customers and/or on the day-to-day work you do with them. This will influence the acceptance of your project. Please note that the “Description of request” section can only contain 2000 characters (1000 in each text box). Please note that projects will be reviewed by a committee made up of representatives from CISSS de Laval and the Foundation.
It is also very important to discuss your project with your supervisor. He or she will be able to determine whether the equipment you are proposing can be purchased from the CISSS budget earmarked for asset replacement.
Conditions:
It is important to understand that the Foundation's role is to improve the care and services offered at CISSS. Please note the following restrictions before submitting your request:
- Any employee working in one of the CISSS de Laval facilities can apply for Loto-Voyage funding (including Jewish Rehabilitation Hospital, CHSLDs, Centre de réadaptation Louise-Vachon, Centre jeunesse, etc.).
- Applicants must demonstrate the project's impact on beneficiaries (and if possible, document it: statistics, needs, etc.).
- Projects submitted may include medical equipment, teaching or intervention materials, or concrete projects/workshops/etc. to improve the quality of life of beneficiaries.
- No project totalling more than $3,000, taxes included, will be considered by the evaluation committee.
- Requests to replace furniture, such as chairs in a waiting room, are not eligible.
- No projects related to training activities will be accepted, as the Foundation awards training grants to nursing staff and members of the Multidisciplinary Council. Contact us for further details.
